CWA MENTORING PROGRAM - APPLICATION

APPLICATION TO BE A MENTOR

Please complete this form and submit it. A copy will be sent to the Mentoring Committee Chair and to the CWA webmistress. Mentors will be listed on the CWA website, and potential applicants will contact you directly. You are not obligated to mentor anyone who asks; it's always your decision. When you arrange for a mentorship, please notify the Mentoring Committee (with the name of the applicant and the dates of the mentorship) so we can track the program. Your comments are welcome.
